Dr. Hitchcock and her team routinely share the PeraHealth scores and graphs with  patients and their families to visualize gradual ongoing deterioration, helping them to better understand the condition of their loved one. Often, this is difficult for the family to perceive which complicates palliative care decision making, however “being able to see graphs for multiple visits with a continually lowering score and trajectory paints a comprehensive picture of health” per Dr. Hitchcock.
